Evidence You'll Need

• Written notes from every appointment, in date order.
• Treatment plans, quotes and consent forms you signed.
• Referral letters to specialists, hospitals or hygienists.
• Correspondence between you and the practice.

Peri-apical films, panoramic (OPG) images, CBCT scans and before-and-after photographs. If your practice used intra-oral cameras, ask for the images.

• A short diary of pain, symptoms and time off work.
• Receipts for corrective treatment, medication and travel.
• Photographs you took of visible injuries or swelling.
• Notes of what you were told by the dentist, and when.

You have the right to a copy of your dental records under UK data-protection law. Practices must respond within a calendar month and cannot charge a fee for a first request. Our team can request them on your behalf.
